Cancellation Policy
Effective September 1, 2024, the CFC Cancellation Policy outlines that all clients are allowed up to 3 non-billed, advanced-notice cancellations (minimum of 24 hours’ notice) per calendar trimester. Click the link above to download a copy of our policy with additional details and caveats.
Note: If cancellations occur on one of the following Federal holidays, they will not be counted towards a client’s trimester allotment: New Year’s Day, Birthday of Martin Luther King, Jr., Inauguration Day, Washington’s Birthday, Memorial Day, Juneteenth National Independence Day, Independence Day, Labor Day, Indigenous People’s Day, Veterans Day, Thanksgiving Day, Christmas Day.

Authorization to Exchange, Obtain, or Release Information
Commonly referred to as a “Release of Information” form or “ROI”, this form lets us know your preference for sharing protected health information.
Drop Off /Pickup Policies & Authorization
If your child or dependent is receiving services at the CFC, we ask that you complete this form so our team can ensure their safety and provide top-quality care.
Credit Card Authorization
If you’re unable to use our portal to update your payment information, then you may use this form as a backup option.
